Need an image of a long course timing slip

I've got a log book I use with my car.  One of the sheets is for each run, where I copy the info from the timing slip.  (Since they don't do well with heat, and are kind of small, they don't always survive . . . )

Anyway, I'd like to print up some log sheets for some friends that run the long course.  Can anyone point me toward (or maybe scan up :-) ) a long course timing slip?  I like to lay out the form like the slip so it's quick and easy to enter the info.
