It looks like early CB750 Honda, that has had the mudflap added (or JPN version?). The left side fender/fork mount was different from the right because of the early disc brake, if I remember correctly. I kind of remembering putting those together out of the crate, when I was working at a Honda dealer in the fall of 1969.
