medical form

In the SCTA rulebook it says all drivers need to submit a medical form.  Is this true for El Mirage, and if so is it this form:http://www.scta-bni.org/SCTA-NewWeb/Bonneville/DRIVER%20MEDICAL%20FORM.pdf

Anything I need to have filled out by my doctor?  It take two months to get an appointment at my doctor (hooray HMO) so i'm trying to get this sorted out now.

I think it's more of a "seizure" thing they are worried about. ,Diabetes , Epilepsy , that sort of thing. Drugs control things like that now, but old prejudices still prevail. I was denied flight school in the Navy because my Father had Diabetes. I still managed to get my commercial license otherwise.

Matt,

I noticed nobody answered your question from March. You probably already know, but if not;

Same form used by BNI and SCTA and required at El Mirage the first time you run each year. Fill it out (no Doctor input required) then make a copy and you have both bases covered.
